ADEA PASS Document Checklist
Everything you need to complete your ADEA PASS residency application — organized for your path. Gather these early; the evaluations and transcripts take the longest.

Personal Statement
Your story and why this specialty. Written in the Program Materials section — you can adapt one base statement across programs.
Resume / CV
Uploaded directly to your PASS application. Some programs also ask for a copy sent to the dean’s office.
Institution Evaluation Form (IEF) — the Dean’s Letter
Required for everyone. Covers class rank, GPA, and an ethics evaluation. Submitted electronically by your dental school’s dean’s office.
Professional Evaluation Forms (PEF) — Letters of Recommendation3 required
Minimum 3, up to 5. Your evaluators submit them through PASS — send them their invites early.
Official Dental School Transcripts (foreign)
Upload your foreign dental school transcripts to the PASS portal, and have your credential evaluator send the official evaluation (see below).
INBDE Scores
Official scores sent to PASS. (NBDE Part I/II accepted if taken in 2012 or later.)
ADAT ScoresSome programs
Official. Not required by all programs, but expected by many competitive specialties (ortho, perio, pros). Check each program.
GRE ScoresSome programs
Self-reported in PASS. Required by a handful of programs — confirm on each program’s page before testing.
WES or ECE Credential EvaluationInternational only
A course-by-course evaluation of your foreign dental degree, sent directly to ADEA PASS. Each program lists whether it prefers WES or ECE — confirm before you order, and start early (it takes weeks).
TOEFL (English Proficiency)International only
Official scores for non-native English speakers. Most programs set a minimum — often 90–100 iBT. Send well before deadlines.
